For students: Academic concerns – questions about classroom policies or grading – should be addressed using the Student Grievance/Appeal form. A printable PDF can be accessed HERE. Non-academic concerns – classmate, staff, or faculty interactions, campus safety, or facilities problems – use the same form. To see how the process works, access this flow chart.

Faculty and Staff should consult their supervisors about concerns of any kind. If the issue involves the supervisor, contact Human Resources directly. Maintenance/facility issues can be reported using the ticket system. Login required.
