Access to Records Form - Family Educational Rights and Privacy Act
In order for a family member to have access to your student records, you (the Student,) must complete this form with all persons you wish to grant permission, and turn it in to the Registrar in Student Affairs. We are not allowed to discuss your personal information with anyone until this form is completed.
