To register for a class, please complete the form below.
Prior to entering the training room, the instructor will take the participants temperature with an infrared thermometer. During the training, distancing and sanitation practices will be followed. Masks will be provided by request. Please answer YES or NO for the following questions. (Note: A “Yes” response would prevent the participant from attending the training session).
1.) Do you have any underlying health issues that would make you more susceptible to being exposed?
2.) Have you had a fever in the past 24 hours?
3.) Have you tested positive for COVID-19?
4.) Have you, or have you been around anyone, that has been on a cruise lately?
5.) Have you been around anyone lately that has been sick or had a fever?
6.) Have you been told that you have to self-monitor lately?
