ASU System Information regarding COVID-19 over the counter (OTC) home diagnostic test and reimbursement from group health plans
On Dec. 2, 2021, the Biden Administration announced that under the Families First Coronavirus Response Act (FFCRA), COVID-19 over-the-counter (OTC) home diagnostic tests will be available to individuals at no cost or through reimbursement from group health plans.
Beginning Jan. 15, 2022, at-home COVID-19 testing will be eligible for reimbursement to Arkansas State University System health plan participants. Per federal regulation, members will be limited to reimbursement for eight at-home COVID-19 tests per person per month.
Reimbursement is limited to at-home tests approved for emergency use by the federal government. Approved tests include:

Members requesting reimbursement for purchased test kits must complete a reimbursement form, which can be accessed here. Members must also include a copy of the receipt to satisfy proof of purchase of a qualifying at-home test.
Completed forms, with supporting documentation, should be submitted to the Evidence-Based Prescription Drug Program (EBRx) at this for processing:
